The mission of HUGS is to prevent child abuse and neglect by providing education and support to strengthen families. Through Healthy Families, HUGS provides services to pregnant women and continues to serve families with children up to age 5. The care coordination includes assessment, education and referrals in targeted zip codes in West Palm Beach, Riviera Beach, Lantana, and Boynton Beach. The Healthy Start/Care Coordination program provides in-home services throughout all of Palm Beach County to pregnant women and families in need of more intensive support services to promote healthy birth and developmental outcomes.

The Healthy Start/Care Coordination program provides in-home services to families in need of more intensive support services to promote healthy birth and developmental outcomes. The Healthy Families program provides in-home services to support and promote strong families. Interventions and goals are developed with families to cope with daily problems, break cycles of poverty, and lead to family self-sufficiency.

The mission of healthy mothers/healthy babies of Palm beach County, Inc. is to improve birth outcomes, promote healthy families, and build healthy communities. The program does this by providing cooperative, community-oriented programs to residents of Palm Beach County. Included in these services are prenatal care coordination to ensure women are enrolled with a health care provider for prenatal care and can assess additional social services, outreach programs for high-risk, hard-to-reach women, and hospital liaisons to inform and register new moms for Healthy Start. It also offers: Teen pregnancy prevention presentations for middle and high school students; conflict resolution classes for elementary school children with behavior problems; and Girl Talk and Guy Talk presentations, teaching 9-16 year olds about puberty, peer pressure and making healthy choices.
Nurture the Future: The Nurture the Future program assists pregnant women, their infants and families with appropriate services to ensure healthy pregnancies and well-baby care. Families are assisted in identifying their needs and with the development of goals for services provision. Care coordinators provide services such as: Education, assessment, community linkage and case management services.

Healthy Start Healthy Families Nurses (Palm Beach County Health Department)

Healthy Start Healthy Families Nurses program consists of skilled nurses and support staff that work as a team to guide mothers through healthy pregnancies and infant care. The program is voluntary and includes health education, care coordination and referral for additional services, when needed. The primary goals of the program are to reduce infant mortality, reduce the number of low birth weight babies and improve health outcomes. Home visits are a key component to achieving these goals, but services are voluntary and information can be provided by phone or mail.
Easter Seals of Florida
Early Steps: The Early Steps program offers support and intervention services to families with infants and children up to 36 months with growth and developmental delays. It provides a wide range of services, including testing, family support plans and education to assist families with meeting their goals. The program model uses a team approach to serve families in their natural environment to increase opportunities for infants and toddlers with disabilities to integrate into the community.

Care coordinators work with families who deal with various issues (such as lack of support, substance abuse, domestic violence, and other psychological and social issues).
The Women’s Health Initiative (WHIN) focuses on reducing and eliminating racial disparities that negatively influence the infant mortality rate and birth outcomes. Services include: Home visiting, care coordination, nutrition, breastfeeding peer education, perinatal depression screening, perinatal depression counseling, well-women care education, goal setting skills, and dental care.
